Treatment of Depression and Anxiety with Medical Comorbidity with Charles DeBattista, DMH, MD

Course Description
Anxiety and depression are common disorders in primary care settings. The typical presentation of the anxious depressed patient is with a variety of physical symptoms. In fact, the more unexplained physical symptom a patient has, the greater likelihood the patient may have anxiety and or depression. Anxiety and depression may also exacerbate a number of co-morbid medical conditions including dyslipedias, diabetes, cancer, stroke and coronary artery disease. In this lecture, the relationship between anxiety, depression and co-morbid medical conditions is examined.

Course Description

Available evidence indicates that the effectiveness of psychotherapy has not improved in spite of 100 years of theorizing and research. The same data shows that consumers continue to harbor doubts regarding effectiveness, with 78% citing a “lack of confidence in the outcome of treatment” as a primary reason for not seeking help. Some attribute the sad state of affairs to poorly trained therapists, others to the continued use of ineffective or unproven treatment approaches. Whatever the cause of the problem, the cure has been more oversight, regulation, and paperwork. And yet, a simple, valid, and reliable alternative exists for maximizing the effectiveness and efficiency of treatment. The approach is based on using ongoing client feedback to inform treatment decisions. Research conducted at multiple sites across a wide range of clients and presenting complaints indicates that clinicians can improve the outcomes of those cases most at risk for failure by as much as 65% without having to change their preferred treatment approach or learn any new treatment techniques.

This hour-long presentation will describe and explore a paradigm shift in thinking about how best incorporate continuous client feedback into clinical assessment, treatment planning, service delivery, and the quality improvement process. The steps and challenges involved in converting a system to this approach including clinical application at the individual case level, aggregate data collection, evaluation and reporting methods, will be described and explored.

Strategic Treatment of Depression with Michael Yapko, Ph.D

Course Description

Depression is the most common mood disorder in the world, a problem that is still growing steadily across all age groups. Depression is also one of the more treatable disorders, as long-term therapy and follow-up studies indicate. Despite the overstated and oversimplified view that suggests depression is caused by a neurochemical imbalance in the brain, the clinical and research evidence make it quite clear that depression is about much more than just “bad chemistry.” In fact, depression can have many causes, and there are many factors, including psychosocial ones, that can influence its course and prognosis.

Medications alone are not and can never be an entirely adequate reply to the growing rates of depression for reasons that will be explored in this brief clinical training. “Adjusting” individuals chemically while individuals, couples and families face life challenges they are not adequately prepared to meet suggests the problem is always in their biology and not their circumstances. Furthermore, for every person who gets appropriately medicated, many more sink into depression that is either undiagnosed or inappropriately untreated. We as health professionals can do a great deal to address this issue much more realistically.

Our knowledge of depression has greatly expanded in recent years, firmly establishing the essential role of psychotherapy in treatment. Simply put, there are things therapy can do that no medication will ever be able to do. In this presentation, we will focus on treatment issues and approaches most relevant to therapy. We will broaden the simplistic view of depression as only an individual biologically based disorder in order to recognize how depression is often a direct response to interpersonal hurtful life situations that people don’t cope with very well. This presentation reaffirms the value of good psychotherapy. A primary goal is to get clinicians re-energized in addressing depression in ways that empower them as well as their clients.

Treating Complex Psychological Trauma with John Briere, Ph.D. *

Course Description
When trauma exposure involves early, repetitive, interpersonal maltreatment, or when there have been multiple and prolonged traumas in adulthood, the outcome may involve not only classic posttraumatic stress, but also dysfunctional relationship styles and schema, affect dysregulation, overdeveloped avoidance responses (especially dissociation, tension reduction behaviors, and substance abuse), and a variety of other affective and behavioral outcomes.

Drawing on the latest trauma research and theory, Dr. Briere describes a non-pathologizing, developmentally-informed therapy for these complex posttraumatic presentations. This approach incorporates both cognitive-behavioral and relational principles to support (a) the processing of implicit and explicit traumatic memories through carefully titrated exposure, (b) the development of increased self-capacities (especially identity and affect regulation), and (c) the reworking of activated relational schema and other implicit memories within the therapeutic relationship.

Pediatric Bipolar Disorder: Early Identification and Intervention with Kiki Chang, M.D. *

Course Description
Pediatric bipolar disorder presents a unique challenge to the clinician. With the growing incidence of diagnosis and poor outcome of patients with early-onset bipolar disorder, early identification and intervention becomes paramount. Over 60% of adults with bipolar disorder report that their symptoms began in childhood or adolescence. If these early presentations could be identified, early treatment and preventative interventions could lessen the impact of and perhaps prevent the full expression of bipolar disorder in these patients.

Neuroimaging and genetic studies have begun to reveal the biological underpinnings of bipolar disorder. These findings might one day be used clinically to predict those at highest risk for bipolar disorder development. Until then, other factors such as symptomatology and family history provide such clues. By integrating early identification and collaboration with primary care providers such as pediatricians, children and adolescents could be treated more effectively before they become adults with treatment-resistant disorders.

Optimizing ADHD Treatment: Subtypes and Comorbidity with Russell Barkley, Ph.D. *

Course Description
Attention deficit hyperactivity disorder (ADHD) occurs in as many as 7-8% of children and 4-5% of adults. While three types of ADHD are currently recognized in the DSM-IV, research suggests there may be only two such subtypes and that one, having predominantly attention problems, may be qualitatively different from the other type having problems with inhibition and self-regulation. These subtypes may have important differences in treatment response as well. ADHD is highly associated with a risk for other mental disorders, with over 80% of clinical referrals having at least one other disorder and more than 60% having at least two other disorders besides ADHD. During the past decade, research and clinical experience have shown that the presence of the second and third disorder have important implications for both understanding the nature and impairments in a clinical case but also for response to prevailing treatments. This course will review the most likely disorders associated with ADHD, the risks they pose for such cases, and the probable impact they may have on the selection of various treatments for case management.

Such issues are woven into the psychosocial makeup of clients and patients and must be considered when evaluating treatment, and planning for interventions. Psychopharmacology for Depression Made Simple with John Preston, Psy.D. *
Recent advances in clinical psychopharmacology have greatly assisted in the effective treatment of depression. This one-hour lecture discusses the limitations and realistic expectations of antidepressant medication treatment with an emphasis on the new developments in diagnosis of mood disorders. In addition, a detailed discussion of empirically derived treatment algorithms will be presented. This lecture will also address issues related to problematic side effects and effective management of these side effects, common reasons for poor or partial responses, the phases of treatment, and best next-step strategies when typical first-line treatments fail.
